Every QR needs a blank margin around it called the quiet zone. Without it, scanners can't tell where the code ends and the background begins. Standard: at least 4 modules on every side — roughly 4 mm on a 40 mm sticker.

Never crop into the finder patterns (the three corner squares).
Avoid busy backgrounds directly behind the code.
Keep contrast strong — dark modules on a light background scan best. Inverted codes (light on dark) fail on many older phones.
